Campus Forms

 

Please submit request forms to the PEIMS Department with specific information so that we may fulfill the request as accurate as possible. All request will be responded on a first come first serve basis within a 3 day turnaround.  Please request online TRANSCRIPTS to Maria I. Guzman at ([email protected]). Requests that require a data query or a special format must be submitted five days prior to the deadline specified on the report.

 

All PEIMS information is confidential and protected under FERPA Law, so we ask that any information released to the requestor be filed appropriately or shredded.
