The District provides administrative regulations addressing employee leaves and absences to implement the provisions of DEC Local Policy. The request form will not be accepted without the Physician’s Statement of Disability. The district shall require employee to use Family and Medical Leave (FMLA) concurrently with paid leave, including extended sick leave, and with temporary disability leave, if applicable.

In accordance with District Policy DEC Legal and DEC Local, this form shall be submitted by the requesting employee. The request form will not be accepted without the Physician’s Statement of Disability.

Pursuant to Section 22.003(b), any employee may request assault leave if the employee is physically assaulted during the performance of the employee’s regular duties
Zapata County ISD has established procedures regarding injuries at work. The Injury Report should be completed as soon as possible and submitted to the injured employee's immediate supervisor. 
The District has established procedures regarding time and attendance. These procedures are in place to ensure all employee's time sheets are accurate, reconciled and consistent.  
The District has established procedures regarding teacher requested transfers. When it comes to decisions regarding assignments or reassignments, all decisions that the Superintendent makes are in the best interest of the school district. 
As per DBD Local, An employee shall disclose in writing to his or her immediate supervisor any outside employment that in any way creates a potential conflict of interest with the proper discharge of assigned duties and responsibilities or with the best interest of the District. The form shall be used to comply with this requirement. 
Return to Work After Extended Leave.
Employees must provide HR with a doctor's note or Fitness for Duty Statement (Functional Assessment Form) before returning to work. If an employee reposts for duty after an extended leave without release from a doctor, the supervisory should contact HR.
